You shouldn't ground pin 12. It puts a direct short on the output of IC18.
My rubbish website including the redrawn Amstrad schematics and the Sinclair Wiki.

I should have been quicker but missed out on this!
I just found my external version created from a dead Amiga drive years ago. It would look so much better in one of those 3d printed cases
